Assure that all consumers receive appropriate medical/health services as needed and that residents receive on-going, daily-proactive health management.

* Pay is based on experience.

Responsibilities Receive monthly MARs from pharmacy, check them for accuracy, write in any new orders that pharmacist did not include on MARs.Per company policies/procedures, transcribe new medication orders to MARs and/or discontinue medications as ordered by physician.Communicate with pharmacist as needed (i.e., order medications; fax new FL-2 and follow up with phone call if necessary; review pharmacist's recommendations by communicating recommendations to physician, quarterly).Read all admission information and assure that there are no discrepancies regarding medication orders (verify accuracy of new orders).Fax FL-2 to pharmacy and copies of any other orders and assure that Office Manager mails blue copy of FL-2 to DSS for SA residents.Count medications brought to the community.Assure resident has had two-step TB testing.Monitor skin care, nail care, vitals, intake and output of residents.Review blood sugar levels on diabetics.Keep documentation on narcotics given, medication errors and medication reorders and file and keep medical records current.Order equipment/supplies if needed.Communicate with transportation aide regarding new orders for special procedures and assure that transportation aide makes appointment for procedure.  Follow up and obtain written results of procedure and report results to doctor or nurse practitioner and then follow up with any new orders received.Will be responsible for rotating On-Call (May include some nights, weekends, and holidays)Assure monthly PCS sheets are printed, that residents are charted on PCS monthly sheets, and maintain PCS assignments.Assure that residents have quarterly wellness checks by their physician and that Licensed Health Professional tasks are completed as needed.Review standing and medication orders every six months.Renew FL-2's yearly, assure OM sends blue copy to DSS regarding residents with enhanced care and fax care plan and FL-2 to DSS.Renew Care Plans yearly as needed and assure that consents are signed for annual flu and pneumonia shots and mail consents to guardians.Prepare copies of MARs, get charts together and assist doctor and/or NP with physical examinations; fax new medication orders to pharmacy; transcribe new medication orders to MARs and document visit, findings and orders in nurse's notes in chart.Assure that blood work, x-rays, urinalysis, etc. are ordered as needed or prescribed by physician.Obtain orders from physicians for hospice care, set up services and communicate with families, case managers, Hospice and Home Health Agencies as needed.Work to maintain the self-respect, personal dignity and physical safety of each resident.Work cooperatively with all other staff and relate favorably to residents, their families and service providers.Know all emergency procedures and be able to apply them at all times.Be familiar with Residents' Bill of Rights and assist residents in exercising those rights.Notify the ED/Administrator of any concerns of neglect/abuse/exploitation by other staff.Participate in quarterly fire drills.Follow all regulations as required by Federal HIPPA Laws.Obtain and keep current all training requiredObtain two-step tuberculin skin test (first test prior to hire and second test within two weeks of first test)Perform other duties as required by circumstances. Qualifications Must be LPN with long term care experience or RN. Must possess a High School Diploma or GED.Must be able to read, write, understand and follow directions. Must possess good written and verbal communication skills. Must be able to Lift, Push, Pull, and Carry a minimum of 50 Pounds. Must be able to Bend, Stoop, Squat, and reach overhead as needed. Must have at least 2 years of supervisory/management experience in an Adult Care Home or similar setting.Must have a Valid NC Driver’s License and maintain during duration of employment.Must have no substantiated findings on the NC Personnel Registry or Health Care Program and maintain during duration of employment. Must have a clear SBI and DMV record and maintain them during duration of employment.Must have had at least 8 years of driving experience, as this is a requirement of company's liability insurance carrier.Must have a two-step tuberculin skin test (first test prior to hire and second test within two weeks of first test).Must be CPR Certified.Must have C.N.A. or Personal Care Aide Certificate (80-Hour) with Personal Care Aide Training.Must have or obtain 15-Hour Medication Aide Certificate within 30-days of Hire. Must complete Medication Aide Check off within 30-days of Hire. Must have or obtain Medication Aide Certification within 60-days of Check off. Must be able to relate positively to residents and families and to work cooperatively with other employees, and vendors.Must be able to carry out responsibilities in a proactive manner.
