Overview *****INTERNAL APPLICANTS ONLY***** Saratoga Retirement Community is seeking a Resident Care Manager (RN/LVN Case Manager) to join our 5-star rated Skilled Nursing Facility! Our facility, which cares for 94 beds, is looking for an enthusiastic and compassionate Resident Care Manager to support our nursing staff and collaborate with physicians, residents, and families. What You’ll Be Doing: As a Resident Care Manager, your responsibilities will include: Supporting nursing staff and collaborating with the interdisciplinary team, physicians, residents, and families. Completing nursing assessments and advocating for residents. Providing patient education and covering the floor as staffing requires. Assisting with admissions and discharges, attending care conferences, and creating/updating care plans. Additional duties as needed. About You: We are looking for driven and compassionate clinicians with the goal of providing exceptional care to our residents. The ideal candidate will have: A current RN or LVN license. Minimum of 1 year of professional nursing experience in a skilled nursing setting. Experience with admin duties, admissions, skin and wound care, communicating with families, and resolving issues. Specialized certifications are a plus. Flexibility and a willingness to support the Director of Nursing (DON) with staffing needs/shortages. The ability to work independently and make sound judgments regarding resident care. Understanding of state, federal, and local regulations in relation to long-term care and skilled nursing. Current and valid CPR certification. Schedules Available Full-Time: Monday – Friday, with flexible hours.
