Job Title: Resident Social Services Coordinator
Job Type: Part-time (20-25 hours per week)
Job Summary:
We are seeking a Resident Social Services Coordinator to provide general case management, intake, and referral services to residents in need of assistance. The successful candidate will be responsible for evaluating the health, psychological, and social needs of residents, developing individually tailored case plans for services, and periodically reassessing the resident's situation and needs. The Resident Social Services Coordinator will also establish linkages with agencies and service providers in the community and determine/develop the best price for services in personal assistance, homemaker, meals on wheels, transportation, counseling, occasional visiting nurses, and preventative health.
RESPONSIBILITIES
· The Social Coordinator is the first contact for resident assistance.
· Plan and organize programs and activities in a Senior building such as health fairs informational seminars and activities to promote a sense of community.
· Promote resident participation in educational and social programs.
· Assist residents with individual needs and working with the management team to maintain a quality standard of living.
· Establish and maintain a resident resource directory and provide guidance to residents when needed.
· Complete reports and preserve documentation on services provided.
· Assist with housekeeping issues, send notices and provide classes when needed.
REQUIREMENTS
Qualified Candidate must possess:
· Computer Skills (Google, Microsoft, etc.)
· Dedicated Self-starter
· Excellent Communication, and People Skills with an upbeat personality
· Strong organizational capabilities and multi-tasker
· Innovative approaches to problem solving
· Working knowledge of the Microsoft Office Software
· Ability to speak Spanish a plus.
