Residential Care Medical Financial Specialist
**Employment Type:** Full time **Shift:** Day Shift **Description:** **_Position Purpose:_** The Residential Care Medical Financial Specialist position provides assistance and training to individuals with Intellectual Disabilities/Serious Mental Illness in an apartment/home setting. We’re looking for people who have experience developing and mentoring a team, and who believe in a strengths-based approach, and use positive practices to help guide their everyday activities. The Medical Financial is the person that schedules and carries out all medical appointments. You’ll be ensuring the health and safety of up to 12 people served in their home setting. A high level of communication and strong documentations skills are a must, along with strong relationship building with both internal and external providers and resources. **_What You Will Do:_** + Manage medications, take residents to appointments, follow up with doctors’ orders, interact with family members, and communicate with all members of the ISP team. + **Individualized Care Planning:** Develop and implement individualized care plans based on thorough assessments, collaborating with residents, their families, and interdisciplinary teams to ensure personalized and effective support. + **Emotional and Behavioral Support:** Offer empathetic and responsive support to individuals facing emotional or behavioral challenges, employing effective communication and de-escalation techniques to create a calm and safe environment. + **Advocacy and Collaboration:** Act as an advocate for residents, liaising with external agencies, families, and healthcare professionals to ensure comprehensive and holistic care.
