Our Residential Case Manager will work out of our Albany, NY campus and will be responsible for supporting clients in the Residential System while performing duties in collaboration with a clinician.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Arrange and attend home visits Prepare transition plans Coordinate Independent Living Services Act as the primary contact between Schools and Residential Coach family visits Complete various assessments, including FASP and SPR Complete accurate case notes and timely input the notes into Connections database Provide transportation for clients Maintain contact with DSS and family Function as a contributing member of the interdisciplinary treatment team Attend dorm meetings Attend treatment review meetings EDUCATION and/or EXPERIENCERequires a Bachelors degree or commensurate level of education and related experience.
Experience with individuals in care systems preferred.
Drivers license is required for this position.
Work requires 25 50% community travel/visits.
