Residential Coordinator

Job Summary 

 

Work in coordination with the Program Manager in overseeing all day-to-day operations of the program. Provide direct care service to individuals living in a supervised residential setting in a manner that promotes growth, independence, and dignity to those served.

Job Duties/Responsibilities 

 

Assume a leadership role in the ongoing operation of the program by assisting or overseeing the staff scheduling, formal staff supervisions (as assigned), and administrative documentation. 

Teach socialization skills and activities of daily living to person served including the following: household maintenance; self care and hygiene; self awareness and human development; menu planning; food shopping and preparation; money management skills/budgeting; transportation and community orientation; consumer skills; and utilization of community resources. 

Coordinate the implementation of Individual Service Plans (ISP) to meet person served behavioral and treatment objectives. 

Record necessary data, write daily progress notes, and complete all other ISP and program required documentation. 

Assist with access to community, including transportation as needed, to individuals served to access community.  Examples include but are not limited to transporting individuals served to and from day programs, work, medical appointments, to family and friends visits, and other recreation and leisure activities as assigned.  

Coordinate with Program Manager to ensure the quality of care is maintained within the program. 

Ensure consumer safety and assist the Program Manager in monitoring person served medical, legal, and financial concerns. 

Supervise and document person served self-administration of medication as prescribed by a physician and in accordance with agency and program procedures. 

Maintain knowledge of and coordinate with the Program Manager regarding ongoing program issues.  Assisting in development and action steps to be implemented. 

Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk.

Job Requirements 

 

To perform this position successfully, you must be able to perform each essential duty and responsibility satisfactorily.  The requirements listed below are representative of the knowledge, skill and/or ability required.   

 

Knowledge of the theories, methods, procedures, and practices of working with individuals with developmental and/or physical disabilities.  

Ability to comprehend materials related to medication administration, ISP’s, PBS, and ADL programs.  

At least two years of experience working with DD/BI/MH population required.  

Demonstrates proficiency in English, both in writing and verbally. 

Ability to practice safe driving.   

Interpersonal skills. 

Ability to work within Microsoft Office products.  

 

Education/Licensure Requirements 

 

High school diploma or equivalency.  

Certified Nursing Assistant, Associate’s degree, or Bachelor’s degree preferred.  

Must possess a valid driver’s license in state of residence, a satisfactory driving record, and at least 3 years of driving experience.  Ability to show proof of valid driver’s license upon hire and thereafter as requested.   

In instances where employees are asked to use their own vehicle to drive/transport individuals served, the appropriate automobile insurance coverage on their vehicle will be required to be shown upon hire and annually maintained thereafter.   

 

Physical Requirements

Ability to lift, push, pull, and maneuver up to 25 - 75 pounds.  Lifting requirements will increase as the complexity of the residential home increases and depending on the specific needs of person served.  

Ability to reach, bend, stand, sit, and walk. 

Repetitive and substantial movements of the wrists, hands, and/or fingers.   

The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; reading.    

Work effectively in a moderate to occasionally loud work environment.
