Assists the Director of Placement with coordinating admissions, transfers and discharges for the Community Living Services Program.

Assists the Director of Placement with coordinating admissions, transfers and discharges for the Community Living Services Program.
POSITION DUTIES
1. Solicits referrals for existing vacancies in Agency programs.
2. Reviews referrals for residential services and refers individuals in need of services to the appropriate residential teams.
3. Coordinates screenings, transfers, admissions and discharge processes for the Community Living Services Program including:
a. Ensuring that individuals accepted for admission meet admission criteria.
b. Ensuring the completion of all necessary applications (i.e.: Social Security applications, and applications for Care Management), notifications and evaluations prior to admission.
c. Coordinating with Care Coordinators and Community Living Services managers to arrange preliminary interview and pre-placement visits.
d. Ensuring the completion of Community Service Plans.
e. Ensuring the gathering of needed financial documents for each admission
f. Attending team conferences, as necessary, to discuss prospective internal transfers, new admissions and discharges.
g. Assist with scheduling and attending planning and admission/exit meetings.
h. Assists with maintaining updated placement reports
i. Maintaining on-going contact with the local DDSO’s Certified Residential Opportunities Team and C.P’s administrators regarding all vacancies and status of placements.
4. Performs all other duties as assigned.
REQUIREMENTS
1. Bachelor’s Degree in human services or related field, or Bachelor’s Degree and five years’ experience in an MR/DD or related field
2. Knowledge of all pertinent Federal and State regulations.
3. Knowledge of community resources in Metropolitan New York region
4. Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the Agency’s corporate compliance program, including the code of business conduct, the NYS and Federal False Claims Acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews
