Short Description Consistently demonstrates the ability to assess, plan, implement, and evaluate respiratory care of all patients based on patients’ specific age group. Anticipates patient needs and consistently addresses patient concerns. Evaluates and reassesses interventions over time to assure best outcomes for the patientsPlans and implements non-standardized interventions, specifically individual exercise programs in accordance with AACVPR/AHA guidelines and physician recommendationsPlans for the patient discharge by working with the health care team to ensure timely dischargeAugments initial respiratory assessment data including biophysical, psychosocial, environmental, self care, discharge planning factors with experimental knowledge to refine individualized treatment plan.*Consistently documents patient evaluation, departmental data, goals, patient notes, individual treatment plan, outcomes, and patient exercise sessions in a timely manner with a high degree of accuracyObtains compiles, interprets and incorporates patient data into monthly individualized treatment plan for the patient which reflects full range of needs, including physical/functional, vocation, nutritional, psychosocial, and emotional and education needsPerforms rapid observational assessment to determine changing priorities. Monitors patients, reports inappropriate responses. Terminates exercise as needed.
