Nazareth Hospital, a member of Trinity Health Mid-Atlantic, is seeking a Lead
Pharmacist to join our Retail Pharmacy team!
Shift:
40hrs/wk Monday-Friday, weekend/holiday rotations. Per department needs.
:
Accurately fill, bill, dispense and assure delivery of patient and employee medications in accordance with pharmacy policies.
Reviews medication orders for appropriate dose, drug utilization, possible adverse reactions, and drug-drug interactions.
Screens orders and/or patient profiles for allergies.
Clarifies problem medication orders prior to dispensing medications.
Clarifies orders directly with physician.
Performs review of patient medication profiles to provide safe and effective therapy.
Answers telephone calls in a professional manner, takes prescriptions over the phone.
Communicates with all patients (and their families) to obtain necessary information, address patient/family concerns/questions and educate them on their inpatient and discharge pharmacological therapy.
Routinely provides physicians, nurses and patients with proper information for the safe and effective use of medications.
Observes and checks work performed by the technicians prior to distribution as appropriate.
Complies with Federal, State and DEA regulations regarding scheduled medications.
Communicates issues to the next shift/pharmacist.
Alerts manger as needed.
Completes MIDAS reports as necessary.
Regularly attends departmental staff meetings/stand ups, or reviews departmental updates provided electronically or in writing.
Assists in the training of new colleagues.
Requirements
PharmD Diploma
PA Pharmacist License
Authorization to Administer Injectables
