The Retention Outreach Specialist (ROS) is responsible for projecting and integrating the Mission and Core Values of the AIDS Healthcare Foundation (AHF) in the provision of quality medical case management services to Ryan White eligible patients with a focus on patients meeting criteria to be enrolled in the Medical Care Coordination program through the Division of HIV & STD Programs (DHSP) 

 

Under the direction of the Manager of Medical Care Coordination or designee for the Medical Care Coordination (MCC)program, the Retention Outreach Specialist (ROS) will directly engage clients who are at-risk of falling out of care or are lost to care. The incumbent will be responsible for reaching the patients through all available means of communication, including but not limited to phone calls, text messages, emails, physical mail, and community outreach to parks, food pantries, and shelters. The person in this role must have experience directly supporting LGBTQ+ individuals, people of color, people with severe cognitive deficits, people with substance use disorders and people experiencing homelessness. Preferably, the incumbent will have experience in with multiple of these communities responsible for planning, coordinating, implementing and evaluating a specific Patient Retention intervention aimed at finding non-engaged, out of care patients living with HIV/AIDS (PLWHA) within their assigned HCCs in Los Angeles County California (Downtown) or when needed at other AHF HCCs as assigned by manager. The ROS is integrated into the Medical Care Coordination team and works closely with team members in identifying, locating, and re-engaging patients back into HIV care.

Essential Job Responsibilities: 

The ROS will work with the MCC team in locating out of care patients by providing outreach services to PLWHA that have fallen out of HIV medical care.

Conduct medical care retention in a non-judgmental manner. Understand and assist patients in negotiating in a complex health intervention system. Maintain relationships/develop networks of providers to encourage/retain patients in HIV care. Work efficiently and autonomously to complete tasks with a high degree of accuracyAbility to prioritize tasks effectively.Documents accurately in assigned data base and electronic medical record systems within 3 days of encounter per AHF policy. Ability to concentrate and focus on tasks in a work environment that contains distracting stimuli. Ability to be flexible in handling work delegated by more than one individual. Skill in prioritizing work to assure the completeness of work in a timely manner when there are deadlines and heavy workloads and minimal direction Identify clinic patients not engaged in HIV medical care within the past 7 months.Work as an integral part of the medical care coordination (MCC) services team, including participating in team meetings. Act as liaison for newly diagnosed patients living with HIV to ensure expedited entry into HIV medical care. Act as liaison for clinic patients recently released from incarceration to ensure timely reengagement into HIV medical care.Work with out of care clinic patients to identify and address potential and/or existing barriers to engagement in medical care.  Utilize motivational interviewing techniques to encourage patients to engage in and/or reengage into HIV medical care.Obtains and maintains HIV/AIDS disease/treatment general knowledge proficiencyAttends and participates in weekly Case Conferences/HCC staff meetings and other MCC appropriate meetings - presents issues regarding individual member clinical, service, or satisfaction.Advocates for client needs with medical provider, Care Team and community. Other duties as assigned to ensure team goals are met.
