Review and investigate Incident/Accident report forms and review they are legible. i.e. collect data from the charts, gather statements from staff, summarize the investigation in a summary note. Review and investigate I/A reportables. i.e. Collect data from the charts, gather statements from staff, complete a full investigation, write reports for the appropriate ICF/IID reporting department MDCPS, MSDH, MFCU and DRM. Evaluate and review team treatment plans and team meeting notes for correct grammar and spelling. Determine that documents reflect active treatment and compliance with state regulations. Evaluate all incidents for reportable occurrences. Drafts external incident reports for review and submission by the Director of Medical Records/Performance Improvement. Analyzes all incident reports and assigns appropriate category codes as outlined by corporate guidelines. Maintains a log of all Incident Reports Identifies trends and makes recommendations to the Performance Improvement department. Ensures immediate reporting of injuries and/or unsafe conditions to supervisor and completes any related documentation. Participate in treatment team process to evaluate programs and make recommendations to enhance quality of care. Ensure all treatment plans for the PRTF coincide with policies, standards and expectations. Review/revise/edit treatment plan database, as necessary, to maintain regulatory compliance. Audit charts for utilization review with needs identified and notify the appropriate staff members. Assist in the utilization review of PRTF patient charts with recommendations to the treatment team.
