The RN provides care to patients in a clinic setting working as a valuable member of the health care team making independent and dependent decisions under the direction of a provider and in collaboration with a core team that includes Patients, Primary Care Physicians, Health Coaches, Pharmacist, Primary Nurses, along with caseload consultants (Psychiatrist, Psychologist or Behavioral Specialist). The RN coordinates weekly review of caseload to develop plans that provide optimal care to the patient throughout the use of appropriate treat to target protocols. The RN is a co-leader in the core team, and is responsible for collaborating with services, both internal and external to the clinic to facilitate care transition to effectively meet the patient's needs by providing health education with a focus on self-management, prevention and wellness, based on the patient's goals.

Full Time | 64 hours every 2 weeks Variable weekdays 7:30am-5pm (typically the same day off each week)Pay and Benefits:

Associate degree Graduate of an accredited school of nursing. Registered Nurse (RN) licensure (RN) in the State of Minnesota Basic Life Support (BLS) Current American Heart Association (AHA) BLS Healthcare Provider card per established policy.
