This role involves assessing patient needs interpreting clinical data and providing age-specific care according to established policies and procedures. **Key Responsibilities:** + Coordinate total patient care for assigned individuals ensuring a tailored and compassionate approach. + Assess patient data interpret clinical information and deliver care specific to each patient's age and needs. + Demonstrate a strong understanding of growth and development principles across the lifespan. + Support the Mission and Core Values of St. Bernardine Medical Center through professional conduct and high-quality care delivery. + Contribute to the hospital’s strategic plan by participating in quality improvement and process enhancement activities.
