Overview Compassionate Care Hospice, an Amedisys Company, is searching for a RN Admissions Transition Coordinator to join our Clermont team of incredible healthcare professionals. It’s at the Heart of What We Do. *NOTE* RN will be stationed at the South Lake Hospital in Clermont and will assess, assist with admissions from hospital to home, work with hospital case managers and physicians, and assist with current team patients that are in the hospital. Attractive Compensation Package $79,000-$85,000 plus after hours pay. *Compensation may vary depending on location, job-related knowledge, skills and experience. On average our orientation for an R.N is 3-4 weeks, however, is individualized according to your needs and experience. Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team. Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician. Functions as a liaison between disciplines and facilities during the admission process. Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate. Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required. Performs on-call responsibilities and on-call services to patients/families as assigned. Performs other duties as assigned. Qualifications 1+ year(s) of nursing experience in acute care setting(s), ER or fast-paced environment(s). Current RN license, specific to the state(s) you are assigned to work. Current CPR certification. Valid driver's license, reliable transportation and liability insurance. HCHB experience and/or acute care nursing roles a HUGE plus!
