**Responsibilities Job Description** **F** **airview is hiring an RN-Breast Imaging Nurse Coordinator** **to join our Breast Imaging department at our Fairview Woodwinds Campus location in Woodbury, MN.** **Schedule** **:** This position will work a **casual** schedule. The shift length will be **8 hours** on the day shift. Primarily function in the role as a Breast Imaging Nurse Coordinator. Ability to work with patients in an environment with time restraints. Need to be familiar with treatment options from current literature and standards. Ability to anticipate patient needs and take appropriate course of action using approved guidelines. Excellent problem solving skills and communication skills. Clinical Expertise: + Coordinates care for patients and families in collaboration with the multidisciplinary team across the continuum of care with an emphasis on newly diagnosed patients. + Synthesizes assessment data and resulting diagnosis. Plans interventions and expected outcomes to provide patient & family necessary information. + Serves as clinical expert/resource to oncology patients, families, health care team and community. + Assists the patient in coordinating multiple specialist appointments, and provides education for patients. + Coordinates multidisciplinary team conferences. + Communicates with referring health care providers regarding comprehensive plan of care for patient and family. + Works closely with physicians and departments to deliver coordinated care in a timely manner. + Works with multidisciplinary team to develop quality monitors to measure patients’ outcomes and program performance. + Collaborates with Mammotrak and Cancer Data Services on data base management. Education: + Assesses learning needs of patient, family, and health care team and creates, implements and evaluates multidisciplinary educational plan. + Utilizes current research, scientific literature, and advanced practice knowledge when providing education for patients, families, community and staff. + Consults with and teaches patients and families about their diagnosis, disease process and plan of care. + Collaborates with surgeons, medical and radiation oncology regarding specific teaching needs. + Provides formal and informal education for health care team and hospital staff as needed. Consultation: + In collaboration with physicians and provides consultation to influence the comprehensive plan of care for patients, family, community and program areas. + Serves as expert resource to patient family and staff. + Assists patients to navigate through the program and services/procedures needed during diagnostic workup and course of treatment. + Provides phone consultation, advocates for care or referrals to patients, families, other health care professionals and the community. + Serves as Program Liaison to community through a variety of communication sources. Procedural Assistance: + Assess, monitor and assist patients before, during and after invasive breast imaging procedures and identifies expected nursing outcomes. + Documents data and updates the plan of care. + Provides discharge instruction to patients and families, post procedure. + Assesses and monitors patient needs during procedures and identifies expected nursing outcomes. + Administers conscious sedation and other procedural related medications in collaboration of a radiologist or physician. + May assist as a circulating RN during invasive exams performed in Interventional, CT, Ultrasound, Nuclear Medicine, MRI & General Radiology Required: Minimum of 3 years working with treatment of breast disease/oncology nursing.
