100% FTE. The scheduled shifts are Monday-Friday, 8 or 9-hour day shifts (10 hour shifts not available) between 7:30 am to 5:30 pm. No weekends. When onsite, you will work at the Administrative Services Building on Westfield Rd. or the Northport Drive Clinic, both located in Madison, WI.

We are seeking a Registered Nurse (RN) to:
Collaborate with a focused interdisciplinary team of MDs, APPs, Social Workers, Schedulers and other RN Care Coordination (RNCC) nurses to care for patients enrolled in the program. Perform the main duties of the RNCC nurse that include care coordination, triage and case management. Communication with patients/families is via telephone or video communication (no in-home visits at this time). Utilize your nursing skills such as triaging emergent issues, providing resources to clients and their families, initiate/facilitate consults/referrals, update care plans, and work with rehab/LTC facilities to coordinate care. It is preferred that nurses have 3-5 years of adult acute care or primary care nursing experience.At UW Health, you will have:
