Summary

JOB SUMMARY:   The RN Care Coordinator will work closely with primary care physicians and various specialty physicians and services to maximize the health status of patients. This position requires a high frequency of contact with the patient and their caregivers for education of health promotion and self-care skills. The RN Care Coordinator will facilitate the medical care of the patient between all care settings including, but not limited to, inpatient, outpatient, ambulatory, and primary care settings. This role will be instrumental in decreasing the cost of care while increasing the quality of care for the high risk patient.

 

REQUIREMENTS:  Basic professional knowledge; equivalent to a Bachelor's degree; working knowledge of theory and practice within a specialized field.  Education Discipline: Nursing degree, Bachelor of Science preferred; Associate of Science acceptable. Experience: 3-5 years.

 

ADDITIONAL COMMENTS:

Certified Case Manager (CCM) or Accredited Case Manager (ACM) credential preferred. Direct experience in acute care, ambulatory care, or payer based case management strongly desired. Working knowledge of post acute levels of care necessary.
