Job Description
The RN Care Manager is responsible for clinical assessment, member-centered care plan development, ongoing care coordination, quality assurance, and monitoring of services to program members, who may be older adults and/or individuals with intellectual or physical disabilities. This role requires interaction with members and their families with the highest level of respect, compassion, and integrity. The RN Care Manager will also participate in efforts to continuously improve the quality of member care and services, fostering a member-centered culture daily.
ResponsibilitiesAchieve member outcomes and goals through adherence to standards of care and best practices.Identify and/or participate in efforts to continuously improve the quality of member care and services.Foster a member-centered culture on a daily basis.Participate in efforts to enhance work experiences.Actively seek and participate in training and development opportunities.Support the achievement of operational goals by adhering to operational standards, and complying with contractual, regulatory, and policy requirements and expectations.Complete discipline-specific initial and ongoing comprehensive assessments.Conduct home visits and home assessments as needed, consulting with families and other supports for additional information as needed.Use critical thinking to optimize members' health and quality of life to reach goals.Develop and maintain care plans.Educate members and families.Additional Skills & QualificationsRN Licensure in the state of Wisconsin2 years of recent experience working with older adults or individuals with physical or intellectual disabilities in a nursing capacity2 years of community nursingCase management experience required Work Environment
The RN Care Manager will manage their schedule based on members' needs, primarily working day shift hours. The role requires in-home and/or long-term care facility visits with members. While there is flexibility to work from home, access to an office will be available for training and internal meetings. The home location for this position is Milwaukee.
