*The Impact You Can Make* *Team Impact* The Care Manager will be assigned to manage their own patient caseload and may also be required to rotate on-call responsibilities with other Care Managers as needed to ensure 24hour/day and 7day/week care for crisis intervention. The Care Manager will communicate with providers and care team on an as needed basis to discuss changes in patient condition that may necessitate treatment change and will attend regular case review meetings. The Care Manager will work closely with the Interdisciplinary Team to ensure appropriate utilization and provision of resources for patients including referrals to outpatient medical, mental health and substance abuse services. In addition to creating/altering the care plan, the Care Coordinator will ensure that a participant/family values assessment is completed.
