RN Care Manager - Hospice
**Description** This is a part time (32 hours/week) position supporting Hospice patients in Santa Rosa and surrounding areas. **Job Description:** Manages an assigned patient/family caseload from admission through the patient's death. Develops and updates the patient/family Plan of Care working collaboratively with the members of Interdisciplinary Team (IDT). Identifies patient/family care needs, implementing nursing interventions as appropriate. Evaluates the results of interventions and adjusts the Plan of Care accordingly. Demonstrates knowledge of costs related to patient care services and manages according to department guidelines. Communicates changes in patient/family condition to the Patient Care Coordinator and team members in a timely manner. Understands the details of insurance reimbursement for nursing services.
