This position is Hybrid (split between field & in-office) Monday through Friday **(NO WEEKENDS or ON CALL)** **Pay** $80,000-$90,000 + Full Benefits Package **Shift** - Monday-Friday - 8am-5pm - No on-call - No weekends **Additional Details** - A lot of field work (mileage reimbursement provided). - When not in field, will be in Chicago office doing virtual visits or also doing work at home (company laptop provided). **Job Summary** The Registered Nurse (Case Management) role involves a mix of field work and virtual visits. The RN is expected to be self-motivated and require minimal oversight. Key responsibilities include supporting census management, reviewing patient data, coordinating transitions of care and post-discharge planning with healthcare providers, as well as potentially conducting outreach to patients by phone.
