Responsibilities Fulfills the role of an RCC completing patient admissions. Serves as a resource to all other RCC's in completing admissions. Leads communication and coordination of care between all members of the care team. Assists all patients and care team members throughout the care model by acting as a patient advocate. Assists market manager in coordination of staffing and ensures appropriate resources are available to support patient care. Ensures that all new RCCs have the appropriate tools and templates to operate. Supports market manager in coordination of clinical and operational workflows and documents those steps. Assists market manager in maintaining data and reporting accuracy. Serves as a subject matter expert in identifying and solving negative trends and hurdles involved in health care system and incorporates solutions for system challenges. Builds and maintains collaborative professional working relationships with all care team members and is available to provide guidance and support during times of urgency or critical needs. Assists as needed in recruiting, interviewing and training of new team members. Performs other duties as assigned. Qualifications Associate of Science in nursing. Current, active RN state licensure. Three to five years of acute nursing experience. Possess a general knowledge of clinical standards and outcome measurement. Reliable transportation and proof of liability insurance. Ability to support after-hours schedules and on-call needs. Preferred Bachelor of Science in nursing. Three months of experience as an acute care RN providing in-home care.
