ChristianaCare is seeking to hire a full-time RN Case Manager for our Heart and Vascular Center in Newark, DE. The selected candidate will serve as an inpatient nurse navigator to support on-site heart failure patient education and discharge planning. The Case Manager will also support the heart failure multi-disciplinary team to maintain heart failure accreditation core measures through performing 72 hour virtual follow up phone calls on all patients discharged with a primary diagnosis of heart failure.

Monday-Friday, day shift
No weekends or holidays
Principal Duties and Responsibilities
Perform ongoing telephonic and/or embedded activities of assessment, problem identification, planning, implementation, coordination, monitoring, and evaluation of patients discharged with a primary heart failure diagnosisCollaborates with patients, caregivers, and members of the multi-disciplinary care team to develop a patient centered plan of care to meet identified patient care goals and outcomesIdentifies issues that serve as barriers to care. Participates in development and implementation of strategies to remove barriers and promote resolution through coordination of a problem-solving processPromote patient wellness and autonomy through advocacy, communication, education, and identification and referral to community resources or other case management programs such as disease management programsCollaborate with healthcare professionals throughout the care continuum to facilitate the transition of the patient to the appropriate level of careDemonstrates knowledge of utilization management and care coordination processes and current standards of care as a foundation for transition planning, complex case management and disease management activitiesConfers with Medical Directors regarding complex cases requiring physician input regarding the treatment plan or physician to treating physician collaborationReceives Super-user training on a variety of programs, IT software, educational presentations, and work with staff in a Super-user capacity to ensure staff compliance.Performs assigned work safely, adhering to established departmental safety rules and practices. Reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, members and visitorsPerforms all other related duties as assigned by Manager/Director.Education and Experience Requirements
Graduate of an accredited school of nursing; BSN within 3 years of hireMaster’s degree preferred.Minimum of 1 -2 years of clinical experience in acute care. Case Management or utilization management experience desirable. Prior cardiology/heart failure experience preferredSpecial Requirements
Active Clinical RN License required.Case Management Certification (CCM) required within 18 months of eligibility.Qualified applicants should apply online and attach a current resume for review.
