Position Summary & Primary Responsibilities:
The RN Case Manager role supports the Director of Health and Wellbeing and the day-to-day clinical operations of the community. The RN Case Manager plans, organizes and coordinates the nursing process to provide excellent care to residents based on The Waters Way of service, presence and partnership. The RN Case Manager serves as a key point of contact for team members, residents, resident representatives and providers. The RN Case Manager is responsible for providing exceptional customer service, utilizing courtesy, dignity, and respect. \n


Clinical Responsibilities:\n\nManages resident care with specific knowledge and experience in assessments, symptom management, crisis intervention, service recovery and family collaboration.\nCollaborates with other health care entities, such as providers or therapeutic disciplines, in the assessment of resident care needs; adjusts the resident service plan to correlate with best recommended care through an interdisciplinary mindset.\nResponsible for the identification of clinical education needs that are outside of routine requirements, such as new resident specific skill competencies.\nResponsible for conducting comprehensive clinical assessments to identify person-centered and holistic approaches to care needs and to promote successful outcomes.\nAssigns and oversees appropriate delegation of services to subordinate team members.\nMaintains appropriate clinical data integrity through best practice documentation standards and appropriate use of data-based systems.\nResponds immediately to unexpected\/emergency situations that arise outside of the daily assignment, including but not limited to: emergency response system calls from residents, security issues, and falls.\nInvestigates and reports suspected resident abuse or neglect per defined procedures immediately.\n\n

Supervisory Responsibilities:\n\nIn conjunction with the Director of Health and Wellbeing, the RN Case Manager provides supervision of the health and wellbeing team, including coaching, counseling, and disciplining employees to promote growth, development, and health and wellbeing.\nMaintains risk management, including medication error and fall assessment and invention.\nCollaborates with the Director of Health and Wellbeing to implement VA reporting procedures and intervention, including MAARC documentation, internal investigation and resolution.\nResponsible for ensuring appropriate application of the resident service plan, including thorough, accurate, and efficiently completed delegated services.\n\n

Education & Experience:\n\nLicensed as a Registered Nurse (RN) in good standing by the state in which the community is located.\nPreferred two years of general nursing experience within a geriatric setting.\nDementia certification or willingness to become certified.\nPreferred previous supervisory and leadership experience #indeedRN\n\n
