Job Description
We are seeking an Extended Care Coordinator / RN Case Manager Assistant to provide comprehensive case management for patients at skilled nursing facilities. This role includes coordination with SNF team members, nursing, rehab, discharge planning, dietary, admission, DON, administrator, and physicians. The position also entails conducting medical necessity screenings and collaborating with the interdisciplinary team to ensure optimal patient care. Responsibilities include accurate medical necessity screening, care coordination, transition planning, communication with patients and families, and maintaining accurate documentation. This role integrates national standards for case management, including utilization management, transition management, compliance with regulatory requirements, and patient education.
