Job Description

We are seeking a dedicated and experienced Case Manager to join our team. The successful candidate will be responsible for planning, developing, assessing, and evaluating the care provided to patients. You will collaborate with physicians, other healthcare team members, and patients/families to ensure continuity, quality, and appropriate resource use. This role involves discharge planning, care coordination, and compliance with regulatory requirements.

ResponsibilitiesPlan, develop, assess, and evaluate care provided to members.Collaborate with physicians, multidisciplinary healthcare team, and patients/families to develop and implement individualized care plans.Recommend alternative levels of care and ensure compliance with federal, state, and local requirements.Assess high-risk patients in need of post-hospital care planning.Develop and coordinate discharge plans to meet patients' identified needs.Communicate discharge plans to physicians, patients, families/caregivers, staff, and appropriate community agencies.Review, monitor, evaluate, and coordinate patients' hospital stays to ensure timely and efficient delivery of essential services.Participate in Bed Huddles and implement recommendations congruent with patients' needs.Coordinate the interdisciplinary approach to providing continuity of care, including utilization management, transfer coordination, discharge planning, and obtaining necessary authorizations/approvals.Conduct daily clinical reviews for utilization/quality management activities based on guidelines/standards for various settings.Act as a liaison between inpatient facilities and referral facilities/agencies and provide case management to referred patients.Refer patients to community resources to meet post-hospital needs.Coordinate the transfer of patients to appropriate facilities and maintain required documentation.Adhere to internal and external regulatory and accreditation requirements and compliance guidelines.Educate healthcare team members on their roles and responsibilities in the discharge planning process and appropriate use of resources.Provide patients with education to assist with their discharge and help them cope with psychological problems related to acute and chronic illness.Report any incidences of unusual occurrences related to quality, risk, and/or patient safety as per established protocols.Review, analyze, and identify utilization patterns and trends, problems, or inappropriate use of resources, and participate in data collection and analysis for special studies or routine monitoring.Coordinate, participate, and facilitate care planning rounds and patient family conferences as needed.Participate in committees, teams, or other work projects/duties as assigned.Essential SkillsCase management experience in an acute care facility for at least 6 months.Minimum of 2 years of acute care clinical experience.Discharge planning experience.Current license as a registered nurse.Basic Life Support (BLS) certification from the American Heart Association.Additional Skills & QualificationsAbility to communicate effectively with patients, families, and healthcare team members.Strong organizational skills and attention to detail.Ability to work independently and as part of a team.Problem-solving skills and ability to think critically in high-pressure situations.Work Environment/Shift

The work schedule is Monday to Friday from 8:30 AM to 5:00 PM, with rotating weekends based on staffing needs. Pay and Benefits

The pay range for this position is $65.00 - $75.00
