Job Description
We are seeking an experienced RN Case Manager Assistant to join our team. In this role, you will provide comprehensive case management for patients at skilled nursing facilities, ensuring coordination with team members across various departments. You will also conduct medical necessity screenings and work collaboratively with the interdisciplinary team to provide care coordination under the direction of a Registered Nurse, adhering to evidence-based practices and regulatory requirements.
ResponsibilitiesProvide comprehensive case management for patients at skilled nursing facilities.Coordinate with skilled nursing facility team members, including nursing, rehab, discharge planning, dietary, admission, DON, administrator, and physicians.Conduct medical necessity screenings and submit for Physician Advisor review.Implement the transition plan based on RN Case Manager and/or Social Worker (SW) assessments.Communicate with the interdisciplinary team during patient care conferences.Manage concurrent disputes and communicate with patients and families regarding the plan of care established by RN, SW, and Physician.Collaborate with physicians, office staff, and ancillary departments.Maintain clear, complete, and concise documentation in electronic systems.Ensure accurate patient demographic and insurance information is maintained.Identify and document potentially avoidable days.Report over and underutilization.Ensure the patient is in the appropriate status and level of care based on medical necessity processes.Communicate clinical data to payers to support admission, level of care, length of stay, and authorization for post-acute services.Advocate for the patient and hospital with payers to secure appropriate payment for services rendered.Complete clinical reviews and promote prudent utilization of resources.Identify appropriate level of care needs and assist with patient transition to the appropriate level of care.Clarify orders and manage patient transitions within the acute care setting.Essential SkillsActive RN license.Experience in an acute hospital setting (200-300 bed hospital) as a floor nurse or clinical role.Discharge planning experience in a hospital setting.Proficiency with HealthConnect or EPIC systems.Additional Skills & QualificationsClinical experience in a hospital.Excel data entry skills.Savvy with data entry and management.Work Environment
This is an inpatient role with a schedule of Monday through Friday, rotating weekends (every other weekend), from 8am to 5pm. The work environment is dynamic and involves collaboration with various departments and team members to ensure optimal patient care and resource utilization.

Pay and BenefitsThe pay range for this position is $60.00 - $60.00
