ABOUT THE ROLE:
The Nurse Care Manager will provide essential medical and clinical case management services to shelter guests participating in the Mercy of Care (MOC) program. Key responsibilities include:
Medication Management: Administering and overseeing medication services.Clinical Assessments: Performing nursing assessments and providing therapeutic interventions.Case Management: Delivering clinical case management and supportive therapy to improve health outcomes.Team Collaboration: Actively participating in the multidisciplinary treatment team to develop individualized treatment plans.Record Keeping: Maintaining accurate clinical records in eClinicalWorks.This role ensures compliance with applicable laws, regulations, and Pittsburgh Mercy’s policies and standards, fostering compassionate and comprehensive care.
