RN Case Manager
Job Title: Nurse Coach openings that are 100% remote! Must have Case Management Experience as well as experience working in a call queues! This is a Tuesday to Saturday position starting in March 2025!

Job Description

The Post-Acute Care program helps to reduce hospital readmissions of patients by providing education and coaching to assist patients in their transition home after a hospital stay. This remote position utilizes clinical expertise to manage assigned Post-Acute Care patients' transition from acute care settings to home through telephonic outreach. The Nurse Coach will provide education, coaching, and care coordination to achieve the goals of the Post-Acute Care Program. The Nurse Coach will engage patients, administer assessments, and work closely with home health agencies and physicians to facilitate education and adherence to healthcare goals and care plans. The role includes participation in performance and operational improvement activities and operates under moderate supervision.

ResponsibilitiesManage the transition of assigned Post-Acute Care patients from acute care settings to home using telephonic outreach and teaching methods.Work within an inbound and outbound patient telephonic engagement model.Educate, coach, and influence patient and caregiver behavior to achieve care plan goals.Engage Post-Acute Care patients in the program and perform assessments to reduce readmissions.Document all interactions, guidance, and interventions in provided applications while maintaining documentation guidelines.Communicate customer service and provider issues to supervisors for resolution.Participate in ongoing case rounds and quality assessment/improvement activities.Implement and maintain operational processes to ensure compliance with company policies, legal requirements, and regulatory mandates.Participate in special projects and perform other duties as assigned.Essential Skills5 years of nursing experience, including 1-2 years of acute nursing experience (e.g., emergency department, intensive care unit, critical care unit, med-surg).2+ years of case management, triage, or telehealth experience (telehealth case management a plus).Strong communication skills for continuous phone interaction with members/patients.Ability to navigate multiple systems/screens.Telehealth/remote work experience.Excellent communication, problem-solving, and decision-making skills.Ability to manage and prioritize tasks effectively.Demonstrated coachability and ability to learn new Electronic Medical Record systems.Work Environment

This position is 100% work-from-home with all necessary equipment provided. Training will be 7 days long and is mandatory without exceptions. After training, the schedule will be 5x8 hour shifts from Tuesday to Saturday, starting at 8am EST. The role includes 1 hour for lunch and the person breaks. Candidates must be comfortable working on an inbound call queue and sitting/working at a computer for 8 hours/day.

Pay and Benefits

The pay range for this position is $38.00 - $40.00/hr.
