Job Type: Contract-to-Hire (6 Months)

Work Schedule: Monday to Friday with rotating weekends (every other weekend). Start time 07:00 AM to end time 3:30 PM.

Job Description: Kaiser is seeking a dedicated RN Case Manager Assistant to provide comprehensive case management for patients at skilled nursing facilities. The role involves coordination with SNF team members, nursing, rehab, discharge planning, dietary, admission, DON, administrator, and physicians. Responsibilities include telephonic case management for the Special Needs Department if needed, conducting medical necessity screening, and collaborating with the interdisciplinary team to provide care coordination under the direction of a Registered Nurse. This position ensures compliance with evidence-based practices and regulatory requirements.

Key Responsibilities:

Payrate: $48 - 52/hr. Provide utilization management of patients in SNFConducts SNF authorization for billing Inputs the billing referrals in EPIC EMR Conducts discharge planning on skilled patientsSupports physicians managing the SNF patientsManages quality / care experience of the patientsConduct accurate medical necessity screeningProvide care coordination and implement transition plansCommunicate effectively with the interdisciplinary teamManage concurrent disputesMaintain clear documentation in electronic systems

Must Have:

RN licenseDischarge planning experienceExperience in an acute hospital setting (200-300 bed hospital)HealthConnect or EPIC experienceExcel data entry

Work Environment: This is a fully on-site position in Anaheim, California.

Pay and Benefits

The pay range for this position is $55.00 - $65.00/hr.
