Start time 07:00 AM to end time 3:30 PM. Job Description: Kaiser is seeking a dedicated RN Case Manager Assistant to provide comprehensive case management for patients at skilled nursing facilities. The role involves coordination with SNF team members, nursing, rehab, discharge planning, dietary, admission, DON, administrator, and physicians. Responsibilities include telephonic case management for the Special Needs Department if needed, conducting medical necessity screening, and collaborating with the interdisciplinary team to provide care coordination under the direction of a Registered Nurse. This position ensures compliance with evidence-based practices and regulatory requirements. Key Responsibilities: + Payrate: $48 - 52/hr. + Provide utilization management of patients in SNF + Conducts SNF authorization for billing + Inputs the billing referrals in EPIC EMR + Conducts discharge planning on skilled patients + Supports physicians managing the SNF patients + Manages quality / care experience of the patients + Conduct accurate medical necessity screening + Provide care coordination and implement transition plans + Communicate effectively with the interdisciplinary team + Manage concurrent disputes + Maintain clear documentation in electronic systems Must Have: + RN license + Discharge planning experience + Experience in an acute hospital setting (200-300 bed hospital) + HealthConnect or EPIC experience + Excel data entry Soft Skills: + Effective communication + Collaboration + Attention to detail + Problem-solving + Advocacy for patients Work Environment: This is a fully on-site position in Anaheim, California.
