RN - Case Manager ID 2025-23576 Category Registered Nurse specialties - Case Manager Facility DOD / MTF - California - Travis Air Force Base Job Description Spectrum Healthcare Resources has a potential need for a Registered Nurse – Case Manager at David Grant Medical Center, Travis Air Force Base, CA. The Nurse will provide case management services within the Integrated Disability Evaluation System. This is a full-time, on-site position, working Monday through Friday, 7:30am-4:30pm.RequirementsAssociate Degree in Nursing from an accredited program.Current, full, active, and unrestricted RN license.At least 3 years of experience within the last 4 years working as a registered nurse case manager.Current Basic Life Support (BLS) certification.Possess one of the following certifications:Certified Case Manager (CCM)Certified Disability Management Specialist (CDMS)Certified Rehabilitation Registered Nurse (CRRN)Certified Occupational Health Nurse (COHN) or COHN-SAdvanced Certification in Continuity of Care (ACCC)Certified Rehabilitation Counselor (CRC)Nurse Case Manager (RN-NCM)Care Manager Certified (CMC)DutiesParticipate in all phases of the Case Management Program (CMP) to ensure established CM standards of care are met.Provide nursing expertise in assessment, planning, implementation, coordination, and monitoring of case management processes.Develop treatment plans that ensure continuity of care and optimal wellness outcomes.Liaise with community agencies and organizations to ensure seamless care transitions.Educate patients and caregivers on their care plans.Facilitate interdisciplinary team meetings and participate in discharge planning.Ensure accurate data collection and documentation.Conduct peer reviews and quality reviews as required.Travel outside the local MTF may be necessary (reimbursable as applicable).Strong organizational, communication, and problem-solving skills required.Proficiency with Microsoft Office applications and familiarity with military healthcare systems.
