The Care Manager (Acute Care) I (CM I) role must optimize and integrate the interface between clinical and financial systems supporting positive health outcomes. Responsible for the assessment, integration and facilitation of patient discharge plans focusing on the coordination of care for the medium to high risk patients across the healthcare continuum. Goal is for discharged patient to possess a “reasonably safe discharge plan”, engaging support systems if patient allows and are available. Ensures the delivery of cost effective, outcome-oriented and quality of care within an expected length of stay. Manage patient care to prevent fragmentation and duplication of services. Works in collaborative practice with the physician and other members of the health care team to meet patient specific and age related needs linking cost and quality to patient care. Role ensures the care is delivered at the “right” level of care and setting.
