RN Case Manager - Full Time - Remote in Michigan
**Employment Type:** Full time **Shift:** Day Shift **Description:** Trinity Health Senior Communities is hiring a dedicated **Registered Nurse (RN) Clinical Case Manager** for our Skilled and Sub-Acute Rehabilitation Communities, where you'll play a crucial role in ensuring high-quality, patient-centered care. In this role, you’ll work closely with an interdisciplinary team, physicians, and insurance providers to support seamless transitions of care, oversee admissions, and guide patients and families through complex healthcare processes. You’ll assess patient needs, develop individualized care plans, and facilitate access to necessary services. ***Candidate must be Michigan based*** *** Position is remote however some travel may be required*** **Responsibilities:** + Perform comprehensive assessments on patient admission and provide ongoing evaluations. + Coordinate admissions and ensure appropriate placement, collaborating with Admissions and the Nurse Liaison. + Act as the primary liaison with insurance companies, obtaining pre-certifications and facilitating concurrent reviews. + Develop and implement alternative discharge plans, coordinating with families and healthcare team members. + Participate actively in care planning meetings to create and monitor individualized patient goals. + Ensure necessary documentation and timely communications with payer sources to maintain patient care quality and continuity. **Qualifications:** + Registered Nurse (RN) with an active Michigan state license; Bachelor’s Degree in Nursing preferred. + Minimum of 3 years of LTC clinical experience, with at least 1 year in post-acute care or case management. + Previous MDS and Insurance Experience + Strong interpersonal, communication, and leadership skills, with proficiency in Microsoft Office Suite. + Ability to work both independently and collaboratively in a fast-paced, patient-centered environment. + Willingness to travel as needed within the community’s geographic region. **Our Commitment to Diversity and Inclusion** Trinity Health is one of the largest not-for-profit, Catholic healthcare systems in the nation.
