We are looking for the Rn Case Manager for Home Health
Full-time position: 09AM - 5PM Monday - Friday

Job Requirements:

• Assessing new patients by gathering information, reviewing diagnoses
• Developing complete plans of care to address the needs of patients
• Monitoring the patient’s needs, including wellness, medical, and specialist treatment
• Communicates with hospital and long-term care facility case managers for care coordination
