RN Case Manager needed for Transition of Care
The RN Case Manager is responsible for coordinatingcontinuum of care activities for assigned patients and ensuring optimumutilization of resources, service delivery, and compliance with medical regime. + Location: Chicago, IL + Weekly pay + benefits offered + Work Schedule: Monday-Friday only + Hours: 8am-430pm Responsibilities: + Reaches out to facility contact to obtain discharge information and associated plans for patients to support their post-transition care + Develops a plan with the patient to meet the identified post discharge needs.The plan may include home visits, accompanying patients to their first visits, or scheduled phone calls. + Outreaches and responds to the patient and/or care giver and introduces self, performs screening and interviews patient to assess needs + Interfaces with care teams to ensure the application of care coordination protocols for screening assessment and care planning + Reports any incidents of suspected abuse, neglect, or exploitation to supervisor immediately upon discovery + Reports feedback to medical home teams according to department policy and procedure Minimum Requirements: + Bachelor's Degree of Science in Nursing (BSN) from an accredited college or university + Licensed as a Registered Professional Nurse in the State of Illinois + 2 years of recent work experience with responsibilities for care coordination across multiple healthcare settings and providers + 2 years of recent experience using electronic medical records, care management systems, or scheduling software + 1 year of recent experience in utilization management or case management + Valid Driver's license and mandatory vehicle insurance or other dependable means of transportation Benefits At Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits: + Competitive pay & weekly paychecks + Health, dental, vision, and life insurance + 401(k) savings plan + Awards and recognition programs *Benefit eligibility is dependent on employment status.
