RN Case Manager Per Diem

Job Summary:

Collaborates with multi-disciplinary teams to assure high-quality, cost-effective care for patients. Coordinates individual patient care throughout the continuum of services.

 

Duties/Responsibilities:

Case find and screen patients to identify appropriate clients for case management. Perform admission, concurrent, focused and retrospective review using established, objective criteria; maintain appropriate record of the review. Verify insurance coverage as appropriate. Interact with external case managers. Assign appropriate level of care; make changes as needed. Perform Quality Improvement and other screens and promptly report findings. Use facilitation and problem-solving techniques to coordinate and manage reduced length of stay, cost containment, customer (patient, physician, and staff) satisfaction, and resource utilization. Assess the patient for: physical, functional, psychological, social, environmental and financial status; mental and psychosocial adjustment to illness. Assess the patient’s informal and formal support systems. Link patient with most appropriate institutional and community resources. Facilitate development of new resources as needed in the service continuum. Provide patients with information on Advanced Directives, community services, Patient Rights, case management, discharge planning and other related information. Provide consultation to outpatients and emergency room patients as requested. Provide information on, and initiate referrals for post-hospital care, including: community agencies, rehab and skilled nursing facilities, adult homes, certified home agencies, home meals, financial assistance and other social service needs. Counsel patients on Medicare, Medicaid and private insurance. Provide services with the ability to identify and intervene for issues that are specific to age, diagnosis and disability of the patient population served, including: psychosocial, developmental tasks and cultural norms and boundaries. Maintain current and accurate documentation in the medical record, case files, records, forms and logs as indicated.

 

Participate in in-services, seminars, reading and other education to maintain professional knowledge of case management, utilization review, discharge planning and social work. Participate in studies and statistical surveys as assigned. Comply with government regulations as applicable. “Perform other duties as assigned.”

 

Required Skills/Abilities (Unit Specific):

Communicate regularly with assigned physician, appropriate nurses, patients and families, to assure appropriate length of stay, cost containment and proper utilization of resources. Handle telephone information with courtesy, accuracy and respect for confidentiality; receive information and distribute messages appropriately. Work effectively and communicate frequently with members of the health care team. Consistently communicate appropriately with patients and their families; speak respectfully and respond to needs or requests for assistance promptly. Provide accurate communication in verbal, nonverbal and written form; utilize effective listening skills.

Education and Experience:

Graduate of an approved RN program, and current licensure in NYS required. Bachelor degree in social work or related health care profession preferred. Minimum of 5 years clinical experience required. Demonstrated competence in discharge planning, social work, utilization review, quality improvement, general health care financial management, and computer skills preferred.

Working Conditions:

Ability to stand for extended periods of time, ability to life twenty (20) pounds. Moving, lifting or transferring of patients may involve lifting up to fifty (50) pounds Assist with weights of more than one-hundred (100) pounds, fine motor skills, visual acuity.

Pay Range:  $35.00-$41.20/hour

Employee salary is based on commensurate experience and other qualifications.
