Schedule: Full-time 40 hours/week Monday-Friday 8a-5p (may be flexible); Occasional evening and weekend events as needed

Location: UChicago Medicine AdventHealth Hinsdale Family Medicine Center, 135 N Oak Street, Hinsdale, IL (Across from Hinsdale Hospital)

The role you’ll contribute:

Coordinates the overall interdisciplinary plan of care for patient. The case manager is responsible for helping to identify the vulnerable patients in our clinic population, help with getting resources to our patients, educate our residents about vulnerable populations, help with home visits and our chronic care grant as well as other opportunities that may arise.

The value you’ll bring to the team:

Creates plan for care across the continuum, integrating patient/family preferences and values.

Monitors patient care through assessments, evaluations and/or patient records.

Advocates for resources and removal of barriers.

Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

Acts as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.

Qualifications

The expertise and experiences you’ll need to succeed:

Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.

Diploma from an accredited school/college of nursing required; BSN strongly preferred or currently enrolled in BSN Program; must be willing to obtain BSN within 3 years from date of hire.

Minimum one year of Case Management or Home Health or Hospice RN experience strongly preferred.
