St. Mary’s Home Care and Hospice is located in Athens, GA and is a member of Trinity Health at Home, a national home care, palliative care and hospice organization serving communities in twelve states. We are a comprehensive, trusted provider of home care and end-of-life care in the sacred place that people call home. A Catholic-based, non-profit organization, we serve patients and their loved ones with home care (skilled nursing, physical/occupational and speech therapy, and medical social work), hospice and bereavement services
**Basic Job Function Summary:**
Under the supervision of the Nurse Leader, coordinates home health care services, labs; and any other equipment, supplies, external services for the agency clients as prescribed by the physician, and/or requested by the client/family, and in compliance with the state’s Nursing Practice Act.
Additionally, responds to incoming and outgoing phone calls, physician communication; documents orders taken in a timely and accurate manner.
This role also serves as backup to the Clinical Manager, when the Clinical Manager is out of the office. Some public/client relations responsibilities.
**Minimum Qualifications:**
• **Current RN licensure in the state of Georgia** , strongly preferring a graduate of an approved BSN education program.
• **Three years or more of Home healthcare experience** , or comparable experience in acute or ambulatory care. Healthcare supervisory experience preferred
• Must have current Driver’s license and reliable transportation to and from work site.
• Must be experienced in documenting in electronic medical records, EPIC experience a plus
• Must be proficient in the use of computer office software.
• Ensures all clinical/client phone calls and service needs (including supplies, equipment) are addressed timely and per policy, processes, ensuring a high degree of patient/family satisfaction and care coordination
• Obtains and accurately/timely documents necessary physician orders for home care services.
• Reviews EMR workflow, manages, reviews Physician Orders, care coordination issues, follows up on QA documentation clarification, requests.
• Provides timely follow-up on physician communication regarding lab results, medication/service changes, or other changes in the plan of care.
