Responsible for coordinating care for cancer patients and coordinating every other week case conferences.Majority phone-based work, communicating with patients to assist with making appointments and guiding their cancer care through Scripps and outside healthcare systems.Responsible for obtaining imaging/pathology from outside Scripps and other hospital/imaging locations, ensuring everything is uploaded to Epic.Put together cases for the case conference and send out to participating providers.Act as the liaison between providers ensuring participation from each department.Provides care coordination and patient education.Assists with procedures.Responsible for planning, implementing, and coordinating services and activities associated with the assigned clinical program.This position acts as an expert clinical resource and communications liaison to physicians, office personnel, nursing staff, allied health professionals, outside agency and community resources, and patients and their families.Through involvement in clinical consultation and direct patient care, assesses and improves the patient care process for the program.This position is responsible for maintaining quality and regulatory standards, maintaining current and accurate policies and associated documents, serving as a link integrating relevant research, best practice and clinical delivery processes procedures and practices, all in a manner which is consistent with the Mission and Vision of Scripps Health.Collects and disseminates clinical quality performance data to site and system committees.This is a benefited, exempt, full time position generally working Monday-Friday from 8:30am-5pm.
