This job introduces the full scope of services offered to clinical staff. Ensures the ongoing updates and education for referral sources. Responsible for the successful clinical coordination of care from the referral source into the home setting. Provides and utilizes clinical expertise and skills to assist referral sources and/or patients with: identification of appropriate candidates for home infusion or home health care, identification of the patients or caregivers ability to provide care, patient education and training regarding therapy and supplies. Assumes responsibility for clarifying expectations of care in the home, medication connection at the hospital/facility in order to expedite the discharge into the home setting as needed.This job description is a summary of the primary duties and responsibilities of the job and position. Education
Required –
Registered Nurse Diploma
Preferred –
Bachelor’s degree in Nursing
Work Experience
Required –
3 years of Nursing experience
Preferred -
Prior nursing experience in an Outpatient & Home Infusion setting
Certifications
Required –
Current RN License in the state of practice
Basic Life Support (BLS) from the American Heart Association
Knowledge Skills and Abilities (KSAs)
Must have computer skills and dexterity required for data entry and retrieval of information.
Effective verbal and written communication skills and the ability to present information clearly and professionally.
Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
Strong interpersonal skills.
Knowledge of core therapy referrals and qualifying criteria by payor source, such as: TPN, Enteral Nutrition and Inotropic Agents for Medicare.
Ability to work with frequent interruptions and changes in priorities.
Must be able to work in stressful situations, while remaining objective and professional.
Job Duties
Develops and maintains core therapy and service line admissions through positive referral source relationships.
Recognizes opportunities for expanding business opportunities with current referral sources/business associates with leadership.
Evaluates current and future clinical and service needs for new and existing accounts.
Coordinates in-services and continuing education programs for referral sources and their support staff.
Collaborates with referral sources (i.e. hospital case managers, home health agencies, physician offices, etc.) to facilitate the coordination of care for new patient referrals.
Coordinates and communicates all discharge planning activities for new patient referrals appropriate personnel and referral sources.
Attends and participates in appropriate office meetings, sales meetings, training programs, regional and national trade shows (prn), and conventions (prn) as directed by leader.
Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
Performs other related duties as assigned.
