Community Case Coordinator Position Summary
Serves in an expanded nursing role to collaborate with Peninsula Regional Clinically Integrated Network (PRCIN) providers. Ensures the delivery of high quality, and cost-effective healthcare services to Medicare beneficiaries requiring comprehensive care coordination support. Assesses, plans, implements, coordinates, monitors, and evaluates all options and services with the goal of optimizing the client’s health status. Ensure beneficiaries receive appropriate care at the appropriate time and in the most appropriate settings. Works with high-risk Medicare beneficiaries in collaboration with providers and the multidisciplinary healthcare team to improve their health outcomes. Integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare.
Community Case Coordinator Position Requirements
Three to five years’ experience in clinical nursing required. Case Management experience required. Knowledge of the basic concepts and principles of care coordination and managed care required. At TidalHealth, team members working at least 36 hours per pay period based on 12-hour shift schedules or at least 37.5 hours for non 12-hour shift schedules and part-time team members working at least 30 hours or more on weekends only are eligible for benefits.
