POSITION PURPOSE: 

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our community services team. As an RN in community services, you will play a crucial role in providing clinical treatment services to the targeted populations.You will work closely with diverse populations, including individuals with chronic illnesses, disabilities, elderly individuals, and underserved communities. The primary focus of this role is to promote wellness, prevent illness, and improve health outcomes through education, advocacy, and direct patient care. As a learned behavioral health professional this position will use face-to-face individual, group psychotherapy, and family-focused behavioral health management techniques to provide assessment and behavioral health supports.

WHAT YOU WILL DO:

Deliver/Observe and educate person served on medication(s)/side effects and useProvides outreach, case management, counseling, advocacy, and other needed services to clients in any environment including: the streets, shelters, jails, hospitals, homes, apartments, personal care homes, state hospitals, inpatient hospital, and rehab settings.Collaboration with treatment team to support techniques that are consumer-first and included trust and respect. Majority (90%) of the work will be conducted in the communityAbility to develop treatment plans based on consumers stage of change for identified problems and needsAbility to implement stage wise treatment interventions as well as exercise sound clinical judgment in routine, urgent, and emergent clinical situationsLink individuals to essential services and resources as clinically indicatedRequires extensive knowledge of Medicaid and Department of Behavioral Health and Developmental Services policies and regulatory guidelines. 

MINIMUM QUALILFICATIONS

Strong clinical assessment and critical thinking skills.Excellent communication and interpersonal skills.Proficiency in computer documentation and electronic health record systems.Flexibility to adapt to changing environments and persons served needs.BSN in nursing and 1 year of nursing experience OR RN license and 2 years of nursing experience. Medical experience a plus. Valid Pennsylvania Registered Nurse license.Valid driver licenseAbility to drive an organization vehicleAct 33/34/74 Clearances (Criminal/Child/FBI Department of Human Services)Pre-Employment Drug ScreenChild Protective Services Law training (CPSL)Working Knowledge and understanding of medication used in treating psychiatric disorders

POSITION HIGHLIGHTS AND BENEFITS:

WORK SCHEDULE: This is an Exempt (salary) position.  Primarily Monday through Friday daylight.  Will rotate on call responsibilities (200/day – 250/holiday day).
