SummaryIn collaboration with other members of the healthcare team, the Complex Care Manager is responsible and accountable for organizing, coordinating, and providing care coordination and care management services to patients within the practice who are most at risk for healthdeterioration, sentinel events, and/or poor outcomes. The care manager works with the ambulatory social worker and community health care worker as an integral part of the complex care management team to ensure that that care for the most vulnerable patients is coordinated and managed. These patients must receive optimal care including acute illness management, chronic disease management, and preventive care across multiple health settings and multiple physicians/providers.

Graduate of an NLN-accredited School of Nursing, BSN preferredLicensure: MA. RN Licensure requiredCertifications: Current AHA (American Heart Association) or ARC (American Red Cross) BLS Provider Certification requiredWork Experience: Minimum of 5 yearsof broad based clinical experienceDemonstrated competency in managing a caseload of ethnically diverse patients across the continuum of care and working effectively withpatients with substance abuse and behavioral health conditions required.Previous Care Management experience strongly preferredPrevious experience working with safety net population preferredPrevious experience working across a variety of settings in the care continuum (including ambulatory and community health nursing) preferred
