POSITION SUMMARY: The Digital Acute Care Registered Nurse is a member of a collaborative team responsible for providing virtual and in-person nursing care to a diverse population of adult patients/clients across the Ministry through a combination of telehealth technology in the centralized command center and hands-on care in the patient's home environment. Provides proactive monitoring, prevention, early intervention, and ongoing care with the goal to optimize quality of life for the patient at home through access to professional acute, inpatient healthcare services. Utilizes evidenced-based care and clinical decision support to guide practice. Performs ongoing communication and works to promote quality of care through collaboration with all service team members, patients, and families. Acts as a patient advocate and resource to other health care providers and community-based resources. The RN demonstrates service excellence and accountability through recognizing the direct relationship between cost-effective and efficient provision of care and services including the impact of access to care. Qualifications REQUIRED QUALIFICATIONS: Education: Associates in Nursing Licenses/Certifications: RN-Registered Nurse American Heart Association BLS Valid current state driver's license Experience: 1 year of recent RN experience in adult acute care population 2 years of experience in a role which required effectively communicating and collaborating with health care professionals Other skills/knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented. PREFERRED QUALIFICATIONS: Education: Bachelor Degree in Nursing Experience: Med/Surg gen med floor work as nurse and IV/phlebotomy skills.
