RN Digital Hospital Admissions Specialist
Overview Pay range for this position is $33.91 - $51.04/hour. Actual pay is based on years of licensure. POSITION SUMMARY: The Digital Hospital Admission Nurse Specialist will identify and screen potential patients who are appropriate for admission into Digital Hospital. Once identified, the Nurse Acquisition Specialist will present the Digital Hospital program to the patient and obtain consent. The Digital Hospital Admission Nurse Specialist is responsible for identifying, assessing, planning, and coordinating, implementing, monitoring and evaluating options and services across the continuum of care for individuals in the Digital Hospital. Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services, as determined by individualized care plans. The Nurse Admission Specialist assures the designation of primary responsibility among team members to each aspect of the plan, avoiding duplication and fragmentation. The Nurse Acquisition Specialist assesses and intervenes to address the complex psychosocial and behavioral health issues and needs associated with hospitalizations and transition plans. Additionally, Demonstrates the knowledge and skills necessary to provide services to the assigned patient population. Flexible work schedule required, including weekend and holiday rotations. Qualifications REQUIRED QUALIFICATIONS: Education: Bachelor’s degree in Nursing Experience 2 year of nursing experience. Experience with chronic illness and medical and community resources Licensure/ Certification RN IL License Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem-solving skills, with the ability to be detail oriented. Working knowledge of a clinical EMR system and Microsoft Office applications. Broad knowledge of chronic illness and medical and community resources. Positive, empathetic approach and ability to communicate effectively to patients and families with a variety of health issues, age groups, and socioeconomic backgrounds. Proven ability to use independent judgement and work independently and efficiently with excellent time management skills. Substantial interpersonal skills to develop and maintain cooperative working relationships with personnel form other departments PREFERRED QUALIFICATIONS: Education: Masters Degree in Nursing Experience: 3 years of experience caring for patients with progressively increasing responsibility Chronic, complex illnesses adult and/or pediatric care management experience.
