RN Director of Clinical Assessment & Intake- Full-Time- Queens, NY
**Req Number** 152009 FlexStaff is seeking an RN Director to Join our dedicated team committed to providing exceptional care to our elderly participants in the Program of All-Inclusive Care for the Elderly (PACE). We focus on quality, compassion, and innovation in healthcare, and we are currently looking for a highly skilled Director of Clinical Intake to oversee our clinical intake processes. As the Director of Clinical Intake, you will lead and manage all clinical activities related to the Intake process for prospective participants. You'll play a pivotal role in evaluating and determining clinical eligibility for enrollment, ensuring high-quality care delivery from the very beginning of the participant's journey. Job Responsibilities: - Drive continuous quality improvement initiatives to enhance staff performance and optimize participant outcomes. - Collaborate with the Care Delivery Department to authorize personal care hours, aligning with participant needs and Medicaid regulations. - Analyze referral trends with the Market Engagement team to increase conversion rates of suitable candidates and enhance enrollment quality. - Provide critical clinical information prior to enrollment for seamless transitions and preparation for complex cases in collaboration with Care Delivery leadership. - Act as the clinical liaison for the Care Delivery team, addressing any concerns or issues arising post-enrollment. - Develop and analyze reports to assess and improve the quality of the clinical intake process. - Ensure accurate, compliant documentation by Clinical Intake Managers and Intake Assessment Managers. - Maintain expertise in Medicare and Medicaid regulations, offering updates to staff and stakeholders. - Engage in staff development by creating and delivering training materials and educational programs. - Manage relationships with contracted agencies, overseeing day-to-day operations and ensuring accuracy in staffing and invoicing. - Develop and implement departmental policies, procedures, and workflows. Qualifications: - Education: BS in Nursing or a health-related field preferred. - Experience: - Minimum of 5+ years in care/case management, disease management, or related fields (preferably within managed care, home care, or community health organizations). - At least 2+ years of experience in Managed Long-Term Care (MLTC) or Managed Care with preparation of the Uniform Assessment System of New York (UAS-NY). - Proficiency in UAS-NY and eMedNY ePACES systems and experience with Customer Relationship Management platforms. - License: Current Registered Nurse license in N.Y. state. - Positive attitude with strong multitasking skills in a dynamic environment. - Proficiency in Microsoft Office, report development, and strong writing skills. - Bilingual in Spanish, Chinese, or Russian is strongly preferred. *Additional Salary Detail The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
