Benefits:                                                                                                           * Weekly pay with direct deposit * GPS time and attendance technology enhances your access to key scheduling and care information * Variety of assignments, procedures, and treatments * Gas stipend  * Equipment provided by company * Full time opportunity/ potential for growth  Responsibilities: * Participates in agency performance improvement activities and continuously staying informed about changes in the field of nursing and home health care * Patient Assessment: Perform comprehensive assessments of patients’ health status, including physical, emotional, and social needs. * Care Planning: Develop and implement individualized care plans based on assessment findings, patient needs, and best practices. * Patient Education: Educate patients and their families on managing illnesses, medications, and overall health maintenance. * Coordination of Care: Collaborate with multidisciplinary teams, including physicians, social workers, and therapists, to ensure comprehensive patient care. * Health Monitoring: Regularly monitor patients’ conditions, documenting changes and reporting them to the appropriate healthcare team members. * Documentation: Maintain accurate and timely documentation in compliance with legal, regulatory, and organizational standards. * Emergency Response: Provide immediate care and interventions in emergency situations and assess when to escalate care. Requirements: * Current unrestricted RN license in the state of NJ * Must be available at least 30 hours on a weekly basis.  * Preferred: Bachelor’s degree in Nursing.
