RN - Group Home Nurse Case Manager - Carmel & Mahopac

POSITION DUTIES AND RESPONSIBILITIES: include but are not limited to the following: 

1. Coordinate the delivery of routine and specialty health care services to address the assessed needs of persons receiving services as assigned and maintain health care profiles. 

2. Assess and establish health care outcomes for persons receiving services. Updates assessments on at least a semiannual basis or as indicated. 

3. Coordinate with the Medical Case Manager Counselors (MCMC) to ensure medical, dental, and specialty appointments, consults, visit documentation, and follow-up appointments are tracked and scheduled according to requirements. 

4. Act as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics, and all other service providers. 

5. Develop a plan of nursing service (PONS) for each individual based on their health care needs, update as needed for changes, and complete staff training to ensure the staff is aware of the individual needs and the staff’s expectations to deliver the necessary care. 

6. Maintain up-to-date health care progress documentation and electronic health records as required by regulation/policy and promptly update as indicated by acute illnesses and/or complex ongoing medical problems. 

7. Evaluate the effectiveness of preventive/ongoing health care and make recommendations/ referrals for alternative care, as necessary. 

8. Conduct general and individual-specific health-related training for persons receiving services, direct care staff, and other team members as required or assigned. 

9. Assign Direct Care staff to perform health care treatments/procedures after considering the complexity of the procedure(s), the skill/experience of the staff, and the health condition(s)/status of the person receiving services. Initially trains the staff in the task/activity and regularly observes/monitors to ensure all treatments/procedures are performed correctly. 

10. In conjunction with the Residential Coordinator, supervise AMAP staff and partner with the Education Department liaison to ensure all AMAP staff are initially certified and tracked to recertify annually. The RN is responsible for completing the documentation and observation for the necessary medication pours for certification and recertification. The RN provides AMAP staff corrective guidance according to the medication policies. 

11. Provide nursing coverage on-site, electronically, or via cell phone between 7:00 am – 7:00 pm, Monday through Friday. 

12. Provide complete and up-to-date handover to On-Call nursing service, including emerging and acute situations. 

13. Maintain and comply with all medical standards outlined in agency policy and OPWDD regulations. Completes follow-up responses for internal reviews, OPWDD, pharmacy audits, and Plans of Corrective Action within the timeline required. 

14. Participate in meetings and committees as assigned, including but not limited to IDT, Day Program, Human Rights, Departmental, and specially arranged meetings/committees. 

15. Coordinate with the pharmacy to ensure medications have current orders, refills are submitted, and medications are available and administered by AMAP-certified staff as ordered. 

16. Review and edit the medication record to ensure the medications are transcribed correctly and monitored as required or assigned. 

17. Responsible for maintaining the medication room/ area and monitoring controlled substances as per the regulations/standards. 

18. Perform all other duties as assigned or indicated, including but not limited to: completing surrogate packets; writing incident/occurrence reports; assisting in developing policies, procedures, and forms; and establishing working systems. 

19. Compliant with all required training online learning in the timeframe assigned 

REQUIREMENTS:

1. Maintain current NYS Registered Nurse license - Graduation from an accredited nursing program - Bachelor’s Degree preferred. 

2. Prior nursing experience and/or experience working with persons with developmental disabilities preferred. 

3. Excellent oral and written communication skills. 

4. Competence in computer skills - Microsoft Office, Electronic medical record experience preferred 

5. Ability to travel to and access multiple locations throughout agency locations 

6. Flexible work schedule with hours and/or days – as approved by the Director of Nursing 

7. Perform all job responsibilities in full compliance with all applicable laws, rules, regulations, policies, and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal False Claims Act, whistleblower protection, and mandatory reporting. Complete corporate compliance training at least annually. Fully cooperate in all corporate compliance investigations and reviews.
