Education
Associate degree from an accredited School of Nursing required; BSN preferred. Bachelor’s degree required within 3.5 years of start date.
Experience
One-year Registered Nurse experience required

Current registration with the Maryland State Board of Examiners of Nurses as a Registered Nurse.Completion of "Healthcare Provider" CPR course, bi-annually.
Physical Requirements
Demonstrates the physical capability to drive in suburban and urban areas to visit partner organization sites, as well as to engage in community events by walking, climbing stairs, and transporting promotional materials using a cart.Patient & Workplace Safety:

Principal Duties and Responsibilities
Supervise the Community Health WorkersWorks with the community health workers and the partners to assure reaching the enrollment goals for the program Conduct blood pressure screenings at community and healthcare events and programs to promote health and wellness.Provide one-on-one conversations with community members to assess their health needs and provide education on healthy habits.Develop and conduct health education programs in a community settingConduct a comprehensive health risk and social determinants of health (SDOH) needs assessment to identify areas of need for community members. Schedule initial primary care appointments for community members who require healthcare services.Provide ongoing follow-up with patients to ensure they are accessing primary care services, SDOH enrollment, and access to healthcare resources.Refer community members to partner agencies that offer SDOH services, and follow up with patients via phone calls text, email, and MyChart to ensure access to SDOH fulfillment.Document patient contacts, referrals, and enrollments in the electronic medical record (EMR) and other software within 24 hours of patient contact. Confirm with Derica the required time for documentation.Represent GBMC HP Baltimore or Gilchrist EMC Baltimore at community partner and other organizational events and programs to promote enrollment in primary care programs.Attend regular staff meetings, Community Partner Coalition meetings, trainings, and other meetings as needed to promote the HERC programAll roles must demonstrate GBMC Values

$63,407.38 - $97,440.12Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
